Course Description

This unit of competency describes the skills and knowledge required in contributing to the care of the client with a chronic
illness by performing nursing interventions that support the client’s needs and assist in maintaining an optimal lifestyle.

2.2Gather and record ongoing clinical data for inclusion in the client’s care plan in line with organisation policy
2.3Contribute information and data on the activities of daily living for the client with chronic health problem for inclusion in discharge plans
2.4Implement client discharge procedures in line with organisation policy and procedures
2.5Accurately gather, document and report changes in client condition to the appropriate health care team members
2.6Contribute to planning the nursing care of a client requiring extended care, including restorative/ rehabilitation care
2.7Identify community based care services for a client with a need for restorative/rehabilitation care
2.8Contribute to planning the nursing care of a client with cancer and/or a life-limiting illness

3.1Identify and prioritise nursing interventions according to client needs as documented in predetermined care plan

Learning Outcomes



Details of Learning Activities

Sessions on diabeties, chronic cardiac and respiratory disease, some of the less common neurological and renal chronic illnesses.  Discussions and lectures on the self help and lifestyle management strategies that have been found useful in managing chronic illness.  Discussions, role play and simulations around palliative care and care of the dying client. 


Teaching Schedule

Many topics spread over a significant time between week 62 and week 99 of the program.  Minimum 80% attendeance is required.

Overview of Assessment

Assessment may include:
Observation in the workplace.
Written Assignments/projects, case study and scenario as a basis for discussion of issues and strategies to contribute to best practice.
Questioning: written or verbal
 


Assessment Tasks

Assessment is via a  case study assignment and clinical performance during final clinical placement.  Case study assignment is required to be submitted before week 96 of the program. If resubmission is required it must be resubmitted within two weeks of the date assignments are handed back in class, irrespective of whether or not the particular student is present that day.  This is because of the tight time frames around finishing the program.  Resubmissions will be notified by student email.
