How to register with ELS

1. Complete the correct registration form below.
2. Have a health care practitioner or provider complete the relevant section of the form.
3. Bring the completed form to your first appointment with ELS or email the form to ELS@rmit.edu.au prior to your appointment.

If your study adjustments include enabling assistance staff, you may need to complete booking forms as directed by your equitable adjustment advisor. 

I am registering for myself

Use this form if you have a disability, long-term illness and/or mental health condition.

A registered health professional will need to complete the relevant section of this form.

When you come to your first ELS appointment, an equitable adjustment advisor will discuss this form with you.

It will help to inform the reasonable adjustments you need for your study and give us permission to communicate with your program about these adjustments.

I am registering as a carer

Use this form if you are the primary carer of a person with a disability, long-term illness and/or mental health condition.

The health practitioner or provider of the person for whom you are the primary carer will need to complete the relevant section of this form.

When you come to your first ELS appointment, an equitable adjustment advisor will discuss this form with you.

It will help to inform the reasonable adjustments you need for your study and give us permission to communicate with your program about these adjustments.

I am studying outside Australia

Complete this form in addition to your registration form

If you are studying at an RMIT campus outside Australia or at an RMIT international partner institution, complete this form along with the relevant registration form.

About enabling assistance staff

To book enabling assistance staff, first make an appointment with ELS to discuss your reasonable adjustment needs.

The enabling assistance staff handbook explains all you need to know about having a note taker, Auslan interpreter or participation assistant. 

If your equitable learning plan includes enabling assistance staff, complete the additional forms below.

They will allow you to book note takers, Auslan interpreters or participation assistants for specific classes, tests for exams.

I need enabling assistance staff during the semester

Complete this form each semester once you know your timetable.

Use this form if you are registered with ELS and require enabling assistance staff for lectures, labs and/or tutorials as part of your reasonable adjustments.

Once you have received the details of your timetable, complete this form and email it to ELS@rmit.edu.au.

I need enabling assistance staff during a test or exam

Complete this form when you have a school-run test or exam.

If you are registered with ELS, your equitable assessment arrangement (EAA) may include enabling staff for tests and exams.

Exams run centrally by RMIT will have enabling staff organised for you.  

For school-run tests and exams, complete the form below with the date and time and email it to ELS@rmit.edu.au.
