Registering with Equitable Learning Services (ELS)
To register with ELS, please complete the forms relevant to your circumstances, then submit your forms to ELS@rmit.edu.au or bring them with you to your initial appointment.
1. Registration and supporting documentation form
Use this form if you have a disability, long-term illness and/or mental health condition. The completed form will be used at your initial appointment with an Equitable Adjustment Advisor to determine reasonable adjustments related to your program of study. This form also provides ELS permission to communicate with your program regarding adjustments to your study.
2. Carer registration and supporting documentation form
Use this form if you are the primary carer of a person with a disability, long-term illness and/or mental health condition. Section B of the form is to be completed by the health practitioner/provider of the person whom you are the primary carer.
3. Singapore student background information form
Singapore students should complete this form, along with the registration and supporting documentation form.
Enabling Staff Assistance
The Enabling Assistance Student Handbook (PDF) explains all you need to know about having a note taker, Auslan interpreter or participation assistant. Prior to receiving enabling staff assistance you will need to sign an agreement form, which is on page 13 of the Handbook.

Semester bookings form
Use this form if you are registered with Equitable Learning Services (ELS) and have enabling staff for lectures, labs and/or tutorials as part of your reasonable adjustments. Once you have received the details of your timetable, fill this form out straight away and email it to ELS@rmit.edu.au.
Exam and test enabling staff booking form
Use this form if you are registered with Equitable Learning Services and have equitable assessment arrangements (EAAs) that include enabling staff for tests and exams. Once you have the details of a test or exam fill this form out and email it to ELS@rmit.edu.au.
