This course provides you with a capstone experience, which will give you the opportunity to integrate, critically reflect on and consolidate what you have learnt in your program. In this course you will explore and practice elements of care coordination, patient education, peer mentoring and leadership. Drawing on the theoretical and practice elements of the program and design thinking for innovation, you will demonstrate capstone capabilities through an individual practice development or mentoring project. This co-developed project will enable you to identify a problem in practice and design and apply an evidence-based response that may be shared with internal and clinical stakeholders. Students will work in a supervision model with academic or selected clinical staff to develop their project demonstrating appropriate time management, leadership and communication skills.
