Appeal Against Assessment – Portfolio Appeals Committee (PDF 61kb 2p)

Appeals against an assessment result must be submitted not more than 20 working days after publication of results.
Please read the relevant University policy and procedure.

Appeal Against a Decision of Student Progress Committee – Portfolio Appeals Committee (PDF 44kb 2p)

Do not use this form if appealing an Exclusion decision. (See Appeal Against Exclusion form below.)
This form must be completed and submitted within five working days of the decision of Student Progress Committee.

Appeal Against RPL and Credit Transfer – Portfolio Appeals Committee (PDF 40kb 2p)

Students may use this form to appeal the outcome of a Recognition of Prior Learning or Credit Transfer application for one or more courses. This form must be submitted within five working days of the decision. Read the University policy and procedures governing student assessment, RPL, and appeals.

This form must be completed and submitted within 10 working days of the decision of the Portfolio Appeals Committee.

Appeal Against Exclusion – University Appeals form (PDF 56kb 2p)

An appeal against a decision to exclude a student from a program must be completed and submitted not more than 20 working days after notification of the exclusion decision.

Appeal Against Special Consideration Determination – University Appeals form (PDF 43kb 2p)

Subject to having submitted a timely and complete application, students may lodge an appeal to the University Appeals Committee against a determination of the Special Consideration Expert Panel not to grant a request for Special Consideration. All appeals must be lodged within 10 working days of notification.

If you are appealing a decision of the Research Committee this form must be completed and submitted not more than 20 working days after you receive notification of that decision..

An Equitable Assessment is an adjustment or alteration to the standard conditions or format of an assessment. It is designed to ensure equitable access to assessment and aims to lessen the impact of a disability on an assessment activity. Application for Special Consideration (PDF 191.6kb 3p)

Please read Adjustments to assessment – special consideration before lodging your application. Special Consideration is a process that enables the University to take account of unexpected circumstances such as illness, injury or bereavement that affect a student's performance in assessment. The University introduced a number of important changes to its Special Consideration policy in Semester 1, 2007. These changes are designed to simplify the application process for students and ensure a more consistent outcome. Students will note that grading consideration is no longer included as a form of adjustment under this policy.

Application for Extension of Time for Submission of Assessable Work (PDF 41kb 2p)

To be used when applying for an extension of time of seven calendar days or less from the original due date for submission of assignments, projects, or essays. Must be lodged no later than one working day before the original submission due date. Not to be used for exams, tests, or lab-based assessments. Refer to Special Consideration Policy for these items and for applications for extensions of greater than seven calendar days.

Changes to student record

Change of Citizenship/Immigration Status (PDF 109.4kb 1p)

Students whose citizenship/immigration status has changed must complete this form.

Change of Personal Information (PDF 124.3kb 1p)

Students who have made changes to their personal information (i.e. address, telephone number, emergency contacts, name) must complete and lodge the signed form at the Hub or post to: RMIT the Hub, GPO Box 2476V, Melbourne, Vic 3001.

Enrolment

Application to Enrol by Proxy (PDF 139.6kb 1p)

This form must be completed by the student and the nominated proxy. This form is only applicable for face-to-face enrolment sessions. Students enrolling online cannot enrol via proxy. International Students commencing an RMIT program are not permitted to enrol by proxy. The form must be presented at enrolment by the proxy.

Application for Late Re-enrolment form (PDF 55kb 1p)

This form must be completed by Higher Education students who have failed to re-enrol by the date specified in their re-enrolment letter. If you receive permission to re-enrol late, you will be liable for a late re-enrolment fee of $175. Follow the late re-enrolment process to apply.

Enrolment form for Student Union Office Bearers (PDF 36.9kb 1p)

This form should be completed by current year Student Union Council Office Bearers and forwarded to the Academic Registrar’s Group. For further information, refer to Enrolment of officers of the Student Union procedure.

Enrolment Variation (PDF 44kb 2p)

Form for processing changes to enrolment - dropping classes and adding classes.
N.B. Under Department of Immigration and Citizenship (DIAC) regulations, international students are required to complete their program within the expected duration. See ESOS and Student Visa Compliance.

Cancellation of Enrolment (PDF 27kb 2p)

Form for allowing students to cancel their enrolment. The date of a cancellation of a program enrolment without academic penalty is set no later than the close of business on the Friday of Week 8 of each semester in the calendar approved by the Academic Board or when an equivalent portion of study has been delivered.

Leave of Absence (PDF 47kb 2p)

Form for applying for a leave of absence. You must attend your School for prior consultation with your program Coordinator or Head of School (or nominee). Leave of Absence approval is at the discretion of the Head of School.

Higher Education

Block Credit Application form – HE (PDF 33.1kb 2p)
This form enables students to provide information to support your application for block credit based on the completion of either formal study or courses provided by recognised professional bodies (e.g. Public Relations Institute of Australia, Association of Certified Practicing Accountants). This form should be completed at enrolment and lodged before the semester Census Date (31 March or 31 August) in your first semester of study at RMIT.

Single Course Credit Application form – HE (PDF 33.8kb 2p)
This form requests information to support applications for single course credit/s based on the completion of either formal study or courses provided by recognised professional bodies (e.g. Public Relations Institute of Australia, Association of Certified Practicing Accountants).

Recognition of Internal Credit Application form – HE (PDF 32.3kb 2p)
Applications for credit for programs completed at RMIT from 1996 within the same career i.e. undergraduate to undergraduate.

Recognition of Prior Learning Application form – HE (PDF 51.2kb 2p)
Provides recognition in RMIT programs for skills and knowledge acquired through assessed study towards formally recognised qualifications, courses provided by professional bodies, work, and other forms of practical and life experience.

Recognition of Prior Learning Assessment document – HE (PDF 45kb 3p)
This assessment document enables you to provide information to support your application for credit transfer based on practical and life experience. Where required, the document should be completed and submitted with the Higher Education Recognition of Prior Learning (RPL) Application form.

TAFE

Credit Transfer Application form – TAFE (PDF 34kb 2p)
This form requests information to support applications for single course credit/s based on the completion of either formal study or courses provided by recognised professional bodies (e.g. Public Relations Institute of Australia, Association of Certified Practicing Accountants).

Recognition of Internal Credit Application form – TAFE (PDF 32.3kb 2p)
This form requests information to support applications for credit for programs completed at RMIT from 1996 within the same career (i.e. TAFE to TAFE). If your application for credit is based on study completed at another institution including Phillip Institute of Technology and Melbourne College of Textiles, please do not use this form, complete ‘Credit Transfer Application form - TAFE (PDF 34kb 2p)’ form available from your school.

Recognition of Prior Learning (RPL) Recognition of Current Competency (RCC) Application form – TAFE (PDF 51.9kb 2p)
Recognition of Prior Learning (RPL) is a process through which you can gain entry to, or credit in, recognised qualifications and skill sets based on competencies gained through experience in the workplace, in voluntary work, in social or domestic activities, or through informal or formal training.

Recognition of Current Competency (RCC) applies if you have successfully completed a unit of competency or module in the past and are now required to be reassessed to ensure that you have maintained your competence.

Cross-Institutional Enrolment – Higher Education only

Inbound

Students currently enrolled in a degree at another recognised Higher Education Provider (HEP), may apply to study single units on a Cross Institutional Inbound basis at RMIT.

Applicants must complete all sections of the forms listed below and submit them to the relevant RMIT school by week 2 of the semester of study.

Forms to complete are:

    • Cross Institutional Inbound form (PDF 38kb 1p) - includes the approval from the student's home university.
    • RMIT Enrolment Form for Cross-Institutional Inbound Students - includes personal information and statistical data forms. This form can be obtained from the RMIT school.
    • Commonwealth Assistance form (CAF) for a Commonwealth Supported Place (if applicable) – obtained at the Hub.
    • Proof of Citizenship (certified copy).

Outbound

Students currently enrolled in an RMIT degree who wish to study at another Australian institution (host institute), may apply to study single units on a Cross Institutional Outbound basis.

Applicants must complete all sections of forms listed below and submit them to the relevant RMIT school.

Application for a concession or exemption of TAFE tuition fees, by local students in places subsidised by the Victorian Department of Education and Training, Office of Training and Tertiary Education ONLY. (NOT Australian full fee paying students or international students.)
The concessions available to Commonwealth Health Care Card or Pensioner Concession Card holders also apply to the card holder’s dependant spouse/child.
Concessions fees will only apply during a semester if the eligibility conditions are met prior to the census date (Semester/Term1: 31 March; Semester/Term2: 31 August).

Application for pre-2008 Student Contribution Rate (HECS) (PDF 45kb 1p)

Application for students who commenced a program before 1 January 2008 who meet the criteria set out on the form.

Application for Refund of Fees (PDF 44kb 1p)

This form must be used to apply for a refund of your tuition fees. It needs to be lodged in person at the Hub on your campus or posted to: The Hub – City Campus, GPO Box 2475V, Melbourne Vic 3001

Application for Remission (removal) of debt in special circumstances form (PDF 63.4kb 6p)

Local Higher Education coursework students who withdraw from a program or course after the Census Date due to special circumstances, or who have been prevented from completing or withdrawing from a course due to special circumstances can use this form to apply to have their: Student Learning Entitlement (SLE) re-credited, FEE HELP balance re-credited, Up-front tuition fee payments refunded, HECS-HELP debt for a course consisting wholly of work experience in industry (WEI) remitted, or Up-front payment of a student contribution in respect of a course consisting wholly of WEI refunded. (For definitions of special circumstances please see page 5 of the application form.)

Sponsored Students form (PDF 118.1kb 1p)

For students who are sponsored by their employer and where their employer is paying the student's fees UPFRONT. This form must be signed by the employer and forwarded to the student's Portfolio Student Administration Office. The form enables Revenue Office to bill the employer direct and also flags the student's record in the Academic Management System as being sponsored or industry funded.

Program change

Application to Transfer Program Plan (PDF 30kb 1p)

You must attend your School Administration Office for prior consultation with your Program Coordinator or Head of School (or Nominee). Program Plan transfer is at the discretion of the Head of School.

Change of Program for International Students form (PDF 73.9kb 1p)

This form is to be used by currently enrolled students (except research students) in pre-university, TAFE and Higher Education programs who have been approved for entry into another program. This form will not be accepted 14 days after the advertised semester commencement date, unless authorised by the University (school).

Dual Award and Double Degree Program Transfer Application (PDF 73.1kb 1p)

Form for students currently studying an RMIT Double Degree or Dual Award program and transferring to the related Single Award at RMIT University. (Australian, New Zealand citizens, or Permanent Residents of Australia only.)

Application for Statement of Academic Completion Higher Education

  • Degree students – log onto the Graduation Database and complete the online form
  • TAFE students – contact your school
  • Master by Research and PhD students – contact the Graduate Research Office

You must apply to graduate before applying for a Statement of Academic Completion.

Archive Transcript Request form (PDF 28kb 1p)

If you studied at RMIT University prior to 1982 or if you are a past student who studied at a college or institute that merged with RMIT, you may need an archive transcript.
A transcript is an official statement of your academic results issued after program conferral (graduation).
