Course Description
This unit describes the skills and knowledge required to determine and address specific issues faced by people who have a chronic disease, in the context of contributing to a holistic approach to support.
The unit applies to workers providing services to people with chronic disease in diverse community services and health contexts.

Assessment 1- Short Answer questions in relation to all of the course content taught - Due : 28th March 2024
Assessment 2 - Practical case study assessed within class and a power point presentation on the chronic you are treating - Due : 24th April 2024

Resubmissions are not automatic and will only be approved at the discretion of the Program Coordinator, after consultation with the teacher and each individual assessment may only be resubmitted only once.
