These forms are to be used to submit nominations for the following School of Health and Biomedical Sciences alumni awards:

Innovation and Enterprise Alumni Award
Rising Star Award
Outstanding Achievement Award

Note that the response to Key Selection Criteria is a 1 page statement max. If you wish to submit additional supportive documents, please include to your application, via email to stemevents@rmit.edu.au, with the subject line: Prizes 2026: SHBS Alumni Innovation.

Please note that this response may be edited by the strategic communications team and may also be displayed in a booklet and/or at an awards ceremony.

Award	Professional/ Academic	Description of the Award	Selection Criteria
Innovation and Enterprise Alumni Award	Professional and Academic	Recognising an individual who has demonstrated exceptional innovation and enterprise, including the development of innovation and/or intellectual property (IP).	Nominators should address all selection criteria: A brief overview of the innovation, enterprise, or intellectual property you have developed The problem or opportunity your innovation addresses How the innovation has been translated into practice (e.g. commercialisation, industry implementation, community adoption, startup development, or policy impact) The impact your innovation has had on organisations, communities, industries, or the broader discipline
Rising Star Award	Professional and Academic	An individual who has graduated 10 or less years ago from RMIT and has demonstrated an outstanding level of professional achievement and community involvement since graduation and/or has been recognised by colleagues and peers for their outstanding leadership and impact.	Nominators should address both selection criteria: Demonstrated excellence, innovation and leadership in their contribution to fields within Health or Biomedical Sciences Evidence of collaboration with and in contribution to local, national or international communities with impact
Outstanding Contribution Award	Professional and Academic	An individual who has made an outstanding, long-term national and internationally recognised contribution to their field of endeavour.	Nominators should address all 3 selection criteria: Demonstrated excellence, innovation and leadership in their contribution to fields within Health or Biomedical Sciences Evidence of collaboration with and in contribution to local, national or international communities with impact Sustained outstanding leadership, innovation and contribution with evidence of wide reaching and exemplary impact
